Medicare and revision surgery

I am 73 years old. I had sleeve surgery in 2014. Lost some weight but gained most of it back due to having to have knee replacement, not exercising and bad eating habits. I now need my other knee replaced but cannot have it until my BMI is 40 or under. That is a weight loss of over 80 pounds. I saw the surgeon that did my sleeve surgery and he agreed to do the bypass. I started the pre surgery tests. The upper G.I. test showed that I have GERDS. My doctor is telling me that Medicare will not cover the revision surgery because of no failure of the sleeve. I am so disappointed and confused. I thought GERDS was cured by having the bypass. He isn’t even turning the request into Medicare because of it being not covered. If anyone can help me figure this out I would so appreciate it. Paying out of pocket is just not possible.
